Terms and Conditions for Focus Psychiatry Clinic

1. Appointment Scheduling:
Patients are encouraged to schedule appointments in advance.

2. Cancellation Policy:
Patients are required to provide at least  2-day notice for appointment cancellations. Failure to do so may result in a cancellation fee.

3. Confidentiality:
All patient information, discussions, and records are treated with the utmost confidentiality. Disclosures will only occur with patient consent or as required by law.

4. Payment and Billing:
Payment for services is due at the time of the appointment. Focus Psychiatry Clinic accepts credit/debit cards only. Billing inquiries can be directed to our office at info@focuspsychiatryclinic.com

5. Insurance:
At this current time,  we do NOT accept any insurances including Medicaid and Medicare. Super bill can be provided so you can submit it to your insurance for possible reimbursement.

6. Treatment Plans:
Treatment plans are developed collaboratively between the patient and the healthcare provider. Patients are encouraged to communicate openly about their preferences and concerns.

7. Medication Management:
Medications may be prescribed as part of the treatment plan. Patients are responsible for adhering to prescribed medications, reporting any side effects promptly, and attending medication review appointments.

8. Code of Conduct:
Patients are expected to conduct themselves respectfully and adhere to the clinic’s policies. Disruptive or inappropriate behavior may result in the termination of services.

9. Feedback and Concerns:
Focus Psychiatry Clinic values patient feedback. Any concerns or feedback about our services can be communicated to our staff or submitted in writing to our clinic manager.

10. Termination of Services:
The clinic reserves the right to terminate services if a patient engages in behavior that jeopardizes the safety of themselves, staff, or other patients.

11. Emergency Situations:
In the case of a psychiatric emergency, patients should contact emergency services or visit the nearest emergency room. Focus Psychiatry Clinic does not provide 24/7 crisis intervention services.

12. Changes to Terms and Conditions:
Focus Psychiatry Clinic reserves the right to update or modify these terms and conditions. Patients will be notified of any changes through appropriate channels.

13. Governing Law:
These terms and conditions are governed by and construed in accordance with the laws of the state of Florida.
